When someone has been released from a hospital stay, they are at their most vulnerable; Lumacare's Home at Last (HAL) program helps clients make the transition from hospital to home safely & enjoyable.
It's more than just a driver; your role would involve:
Picking up clients from Toronto hospitals in a Lumacare’s Home at Last vehicle and driving them to their home.
Assisting clients with settling into their home. This includes personal care tasks and homemaking supports upon arrival. It may include grocery and pharmacy pick up along the way.
Providing caring service and supporting client’s physical and emotional needs as they make the transition back into their home.
Maintaining appropriate follow-up and documentation.
Promoting Lumacare services and the Home at Last program to hospital staff and the community at large.
Requirements for the position include:
PSW Certificate from a certified educational institution with a minimum of 1-2 years working experience.
A valid Ontario “G” class driver’s license with a minimum of 3 years of verifiable insurance (primary or secondary), and clean driver’s abstract is mandatory.
3 Years of Passenger Transportation experience is required (Bus, taxi, UBER, etc.)
Physical ability to perform light cleaning and lifting and basic knowledge of health and safety standards.
Crisis intervention and problem solving skills are required to manage client issues.
Strong computer skills; excellent interpersonal and communication skills are essential.
Current First AID & CPR Certification
Current Vulnerable Sector Police check completed within last 12 months is required.
